Dear Parents,

Thank you for your patience while we process the items we need to get us settled for the end of the school year. Below are the steps to get a refund for school activities, sports, etc. If you have heard information from coaches or teachers, please follow their instructions as well as filling out the form below. After-School STEM should have received information from Mrs. Gibson earlier today, those students should not fill out this form, her refund information can be found here: 9th grade Spain Trip: Please do not submit this form for a Spain Trip refund, we will be in touch soon regarding next steps for you.

1) Access the refund form here:

2) Download the PDF and save it to your computer, phone, or other device. You can then fill out the form on your device, save it, and email the file to: - please fill out one form for each student.

  • If you cannot fill out the form online and do not have access to a printer, you can come to the Jr. High building and pick up a form from the POET desk, Monday-Thursday, 10:00 AM-2:00 PM, or Friday 11:00 AM-2:00 PM. Then give the filled out form to the person at the front desk.

3) Once you have completed the form, you will be mailed a check within 2-3 weeks. If you originally paid online by credit card, the amount will be credited to your card.
